Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to struck-by hazards:
Employees were exposed of being struck-by the load suspended from the
hoist and the
employer failed to establish and follow a program of frequent and periodic
inspections of the
hoist system:
a)At Tuber #1, an R&M Spacemaster II, 2 ton hoist
b)At Tuber #2, Coffing HST027, 2 ton hoist
c)At Tuber #3, Yale KW451090, 2 ton hoist
d)At Tuber #4, Coffing HST022, 2 ton hoist
e)In the Plate Installation area, CM L-152CA, 1 ton hoist
c)At Tuber #3, Yale KW451090, 2 ton hoist
d)At Tuber #4, Coffing HST022, 2 ton hoist
e)In the Plate Installation area, CM L-152CA, 1 ton hoist
f)At Embosser Unwind, CM Valuestar 2 ton hoist
G)At Embosser Rewind, CM Lodestar 2 ton hoist
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to establish and follow a program of frequent and periodic inspections
of the hoist systems
in accordance with American Society of Mechanical Engineers (ASME)
B30.16-2003
Overhead Hoists (Underhung), Sections 16-2.1.1(a) Initial Inspection,
16-2.1.2 Frequent
Inspections, 16-2.1.3 Periodic Inspections and 16-2.1.4 Hoists Not in
Regular Service.

the
requirements
of 29 CFR 1903.19(c)***

Employees in the Tuber Area, at the Embosser and in Plate Installation
area were exposed to
the hazard of being struck-by the load suspended from below-the-hook
lifting devices and the
employer did not proof test, inspect or mark the devices with their rated
capacities prior to
putting them in service.
Among other feasible methods, one feasible and acceptable method to
correct this hazard is
to ensure the below-the-hook lifting devices are proof tested, inspected
and marked in
accordance with American Society of Mechanical Engineers (ASME)
B30.20-2006
