Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing injuries associated with the rollover of a lift trucks without the use of an operator restraint system: (a)Facility Floor: On October 21, 2009 and time prior to, an employee operated a Toyota fork lift (model 7FGU25, serial # 84949) without the use of the installed seatbelts. ABATEMENT NOTE: Among other methods, feasible and acceptable abatement methods to correct this hazard are: 1.To follow ASME B56.1-1993 Safety Standard for Low and High Lift Trucks and require the usage of an operator restraint system at all times. 2.To follow the manufacturers operating manual and wear the provided seat belts at all times when operating the lift trucks. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
