29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Gang Saw Line: On or about December 18, 2017 and times prior, an employee performed maintenance on the gang saw that had multiple energy sources such as 480 volt electrical and 1600 p.s.i. hydraulic.  Procedures used for the control of potentially hazardous energy during the gang saw blade change did not comply with the requirements of 29 CFR 1910.147(c)(ii)(A)-(D).

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) GangSaw Line: On or about December 18, 2017 and times prior, an employee changed the blades on the gangsaw.  The gangsaw has multiple energy sources such as 1600 p.s.i. hydraulic and 480V electrical.  The gangsaw was powered off but locks were not applied to control the hazardous energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  a) Gang Saw Line: On or about December 18, 2017 and times prior,  an employee changed the blades on the gang saw line without applying energy control procedures.  The employee who performed the servicing and maintenance was not  identified and trained as an authorized employee.
