forks without a proper cage designed for that purpose:
(a)Tire Shop: Employee(s) were riding on a Hyster fork lift truck in an
unsafe
manner in that during the operation of getting tires on and off of tire
racks the
employee(s) would stand on a wooden pallet and the operator of the fork
lift
would lift the person to the height where the tires was located and then
place
the tire from the pallet to the rack as high as 15 feet to ground level
without a
proper cage designed by the manufacture as determined on 11-05-2010.
Note: Among other methods, feasible and acceptable abatement methods to
correct this hazard include, but are not limited to the following: Ensure
that
the provisions of ASME B56.1-2004 (or ASME B56.1-2000), Safety Standard
for Low Lift and High Lift Trucks, Section 4.17 Elevating Personnel, are
complied with for the protection of personnel lifted on forklifts.

Abatement Note: Feasible means to abatement. There was a feasible and
useful
method to correct the hazard. Among other methods, one feasible and
acceptable abatement method to correct this hazard is to follow ASME B56.1-
1993 Safety Standard for Low and High Lift Trucks and require the usage of
an operator restraint system at all times and to follow the manufactures
operating manual and wear the provided seat belts at all times when
operating
the lift trucks.
