about 4/13/2010, an employee operating a Case brand forklift was observed
not
utilizing the available seat belt.
Abatement Note:  Among other methods, one feasible and acceptable
abatement method to
correct this hazard is to enforce page 4 of the employers forklift safety
policy manual that states
the following:  "if a seat belt is provided, it shall be worn at all times
while operating the
forklift."

attachment:

about 4/13/2010, an employee was observed operating a Case forklift with an
improperly secured self-dumping hopper. The self-dumping hopper was not
attached to the fork truck to prevent it from sliding off the tines.
Abatement Note: One feasible and acceptable means to correct this hazard
is to follow page one
of the Vestil owners manual that states the following: "Ensure that the
dumper is locked in place
and secured to the fork truck's fork mast before transporting it or
raising it off the ground.
