Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards associated with being caught in machinery:
a) Employees were provided with a Rigid Model 400A portable pipe threading
machine that was
not equipped with a momentary contact device, such as a deadman foot
switch, to control the
power input to the machine as specified in the American National Standards
Institute (ANSI)
standard for pipe tools, B208.1-1982 "Cord Connected Portable Pipe
Threading Machines and
Portable Power Drives-Safety Requirements," Paragraph 4.4.
Among others, one feasible and acceptable means of abatement would be to
comply with the
requirements specified in ANSI B208.1-1982 and provide a momentary contact
device such as
a guarded foot switch to control power to the machine.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the certification of corrective action worksheet), and in addition,
documentation
demonstrating that abatement is complete must be included with your
certification.  This
documentation may include, but is not limited to, evidence of the purchase
or repair of
equipment, photographic or video evidence of abatement, or other written
records.
