Section 5(a)(1) The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing, or were likely to cause death or serious physical harm to employees in that employees were exposed to a potential crushing hazard: a. On or about February 1, 2010, and at times prior, the Taillift powered industrial truck boom, manufactured by Vestil Manufacturing Corporation, was not secured to the powered industrial truck carriage. Among other methods, one feasible and acceptable abatement to correct this hazard is to follow Page 1 of the Vestil Lift Master Boom Operators Manual, which states: "Secure Lift Master Boom to fork truck carriage with safety chain". Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
