Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to falls from elevations:
a)On September 10, 2009 at the site located at 7700 Winoa in Norridge,
Illinois
60706, an employee of Horizon Contractors, Inc. was using an Elliott Model
ECG-4-85 (G85) Hi Reach Aerial Lift truck #635, which tipped over onto the
passenger side of the aerial lift platform. The outriggers on the
passenger side of
the vehicle were not fully extended to prevent the aerial lift from
tipping over.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
comply with the American National Standard Institute (ANSI) for Safety
Requirements for
Vehicle-Mounted Elevating and Rotating Aerial Devices, ANSI A92.2 Section
10.8 (2001),  the
manufacturer's operating instructions of Elliot Equipment Company and to
ensure that outriggers
are fully extended.
**In accordance with 29 CFR 1903.19(d), abatement certification is
required for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase
or repair of the equipment, photographic or video evidence of abatement,
or other written
records.**
