Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the pendant control for the underhung hoist located in the phosphate room was not legibly marked to indicate the purpose of the control buttons, exposing employees to struck-by hazards from the load if it traveled in an unintended direction. a.Hydro Tube Enterprises, Inc. located in Oberlin, Ohio: On or about May 16, 2011, the employer did not ensure the control buttons were legible on the pendant control used for the single girder underhung hoist located in the phosphate room. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with ASME B30.16-2007 "Overhead Hoists (Underhung)" but not limited to: a) Ensure the controls to the remote control pendant are marked to indicate the direction of resultant motion according to Section 16. No additional abatement information is needed for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the hooks and lifting carriage used to lift parts racks in the phosphate room and degreasing room were not labeled as to their capacities, exposing employees to a struck-by hazards if the hooks and lifting carriage were to be overloaded: a.Hydro Tube Enterprises, Inc. located in Oberlin, Ohio: On or about May 16, 2011 the employer did not ensure employees using hooks in the phosphate room to lift racks of parts into and out of the different dip tanks, were labeled as to their capacity. b.Hydro Tube Enterprises, Inc. located in Oberlin, Ohio: On or about May 16, 2011 the employer did not ensure employees using a lifting carriage to lift parts into and out of the degreasing tank, was labeled as to its capacity. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with ASME B30.20-2010 "Below the Hook Lifting Devices" but not limited to: a) Ensure the below the hook lifting device is marked as to indicate the rated load of the device according to Section 20. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation ( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Abatement note: Among other methods one feasible and acceptable abatement method to correct this hazard is to comply with ASME B30.10-2009 "Hooks" but not limited to: a) Ensure the hook is equipped with a functioning safety latch according to Section 10. No additional abatement information is needed for this item.
