Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 29, 2023, the employer did not ensure that employees who serviced and maintained Robot Weld Cell B were trained in the skills required for the safe application, usage, and removal of energy controls. Employees were not trained to isolate and render energy controls safe prior to performing unjamming tasks. b. Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 29, 2023, the employer did not ensure that employees who setup the BLM bender (model ELECT-L; no. 650100301010) were trained in the skills required for the safe application, usage, and removal of energy controls. Employees were not trained to isolate and render energy controls safe prior to performing setup activities. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 29, 2023, employees were exposed to struck-by and crushing injuries in that the robot welder, located at Robot Weld Cell B, was not locked out prior to performing unjamming tasks. The employer did not ensure that energy control application steps were implemented to control the machine's electrical energy sources prior to the servicing task - the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. b. Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 29, 2023, employees were exposed to crushing and amputation injuries in that the BLM bender (model ELECT-L; no. 650100301010) was not locked out prior to performing setup tasks. (model 3CP-HD; s/n 84236-64) flaring machine, located in the End Forming area, was guarded to protect employees from pinch point hazards created by the closing clamp. b. Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 23, 2023, the employer did not ensure that the PHI beading machine (model 3CPV; s/n 3CPV-23), located in the End Forming area, was adequately guarded to protect employees from pinch point hazards created by the closing clamp. c. Hydro Tube Enterprises, Inc. - Oberlin, Ohio: On or about November 30, 2023, employees who operated the tube bender located in the coil cell and used to bend the ends/tails of coils, were exposed to the pinch point created after the bend is made and the die holder/block retracts back to the home position. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees responsible for performing electrical tests and removing fuses in a 480-volt electrical distribution panel for a pump motor located in the phosphate room, were exposed to hazards in that electrical safe work practices were not employed to prevent electrical shock and/or arc flash/blast injuries. An employee was exposed to 480 volts while performing electrical tests within the panel.
