IC 22 08 01 01 2:  The employer did not establish and maintain conditions
of work which were reasonably
safe and healthful for employees, and free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
hazardous energy:
Vertical Turret Lathe (VTL)  reference # B 995 -  The operator control
station(s) are the two clutch lever(s)
located on the right and left front sides of the VTL.  The clutch levers
are approximately at floor level and are
sixty-eight inches apart from each other and readily accessible to the
operator from his/her normal work
position. These clutch levers start the rotation of the turret table and
also engage the brake to stop rotation.
When one control station clutch lever is engaged and moved forward or
backward the other control station
moves to the same location as the first control station.  The clutch
levers operate simultaneously when engaged
by the machinist.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
To develop and implement a process that relocates the clutch levers to a
protected location that allows the
operation of one control station at a time in accordance with ANSI
B11.8-2001 and guard the clutch levers to
prevent accidental start-up.
