General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were likely to cause death or serious physical harm, severe burns and
possibly
death
from the improper design and maintenance of the wire/radiator processing
machine and
related processes:
Wire process machine:
(1) Wire process system produced fine particles of aluminum and was not
provided with
hoods, capture devices, or enclosures which were connected to a dust
collection system with
sufficient suction and capture velocity to collect and transport all the
dust produced.
(2) Fugitive dust present was not removed with approved equipment such as,
conductive non
sparking scoop, brooms, natural fiber bristles, or vacuum cleaning systems
designed to
handle combustible metals.
Dust collector for the wire process machine:
(2) Dust collection system was not dedicated to the collection of aluminum
dust only.  Other
metal dust, including but not limited to copper dust, was also collected
in this system. Dust
collection system was not thoroughly cleaned was not disassembled and
thoroughly cleaned
of all incompatible material prior to and after its use.
(3) Ducts were not fabricated so as to prevent the escape of transported
materials from
leaking flange connections.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE MEANS OF
ABATEMENT WOULD BE TO COMPLY WITH THE GUIDELINES OUTLINED IN
THE NATIONAL FIRE PROTECTION ASSOCIATION (NFPA) 484 "STANDARD FOR
COMBUSTIBLE METAL (2009) EDITION."  SECTIONS 6.3, 6.4, and 6.5.
