Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a struck-by hazard: a) Hilfiger Road, Cherry Flats, Covington, PA, Rig #5: A 6 inch pipe and hose connection used to pump mud at approximately 3,100 p.s.i. was not secured to prevent unsafe movement of the pipe and hose in the event of separation, observed on or about April 21, 2011. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Ensure that each section of high pressure hose is properly secured to prevent unsafe movement in accordance with American Petroleum Institute API RP-54, Recommended Practice for Occupational Safety of Oil and Gas Well Drilling and Servicing Operations Manual, page 20, section 9.13.3, which states: Each section of a high pressure rigid line should be secured using appropriate means to prevent movement should the line fail. Flexible high pressure hoses should be properly secured to prevent unsafe movement. ABATEMENT OBSERVED DURING INSPECTION NO CERTIFICATION REQUIREDATION
