Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to struck-by and crushed-by hazards: (a)On or about 08/22/2010 in Bay 1, Front Bay at Big 10 Tire Store #128, 1210 Decatur Hwy, Gardendale, AL -Employees were operating a Rotary Lift model SPOA9-200, SN AAU01J0547, to lift a Ford 150 truck loaded with tools and equipment and the back-end of the vehicle fell off the lift. Among other methods, one feasible and acceptable method to correct this hazard is to follow safety and operating instructions in the manufacturer's "Operation & Maintenance Manual for Rotary Lift models SPOA7, SPOA9, SPO9" and in ANSI/ALI ALOIM-1994, "American National Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance".
