Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by a Front End Loader:
a) Pawtucket Recycling Associates LLC. A John Deere Front End Loader had
been altered from
its original manufactured specifications when its rearview mirrors were
removed from service.
On or about 04/18/11 An employee was struck & injured by a John Deere
Loader when the
operator had begun to reverse the loader and could not see the employee
behind the machine.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
comply with the manufacturers Operation & Maintenance Manual and provided
proper training.
