Section 5(a)(1) The employer did not furnish employment and a place of
employment which
was free from recognized hazards that were causing, or were likely to
cause death or serious
physical harm to employees in that employees were exposed to a potential
crushing hazard:
a. On or about February 2, 2011, and at times prior, the powered
industrial truck boom,
used to transport slabs weighing hundreds of pounds, was not secured to
the powered
industrial truck carriage.
Among other methods, one feasible and acceptable abatement to correct this
hazard is to
secure the boom to the powered industrial truck carriage with a safety
chain.
b. On or about February 2, 2011, and at times prior, the powered
industrial truck boom was
not properly labeled/tested to ensure that it had the proper load rating
to move slabs.
Among other methods, one feasible and acceptable abatement to correct this
hazard is to
have the boom appropriately tested/labeled or to replace the boom with one
that has been
load tested/labeled.
c. On or about February 2, 2011, and at times prior, slabs both inside and
outside the shop
were not secured to the A-frames.
Among other methods, one feasible and acceptable abatement to correct this
hazard is to
secure the boom to fork truck carriage with a safety chain.
