Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing, or likely to cause, death or serious physical harm to
employees in that employees
were exposed to being struck by falling materials stored in the climbing
assist device:
On or about April 1, 2011, on Altair Rig #7, employees placed BOP bolts in
the
crown area which could fall and strike employees working at ground level
or on
the drilling platform during rip up operations.  No means of securing the
bolts
from falling was used.
