Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards while working from a wooden pallet elevated
in
the air attached
to a Toyota forklift:
(a)Coffee Warehouse, 206 Finley Road, Belle Vernon, PA: On or about
September 27,
2011, employees were working from an unguarded wooden pallet that was
attached to a Toyota
forklift at an elevated height of approximately 12 feet and 10 inches
high, while installing new
racking systems in the Coffee Warehouse.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to comply with ANSI/ITSDF B56.1-2009 Safety Standard for Low Lift and High
Lift Trucks,
Section 4.17.1 and 4.17.3 or utilize an aerial lift or a scissor lift.or
