a. On or about June 7, 2011, at the electrical platform, employees
utilized locks and
hasps intended for control of electrical energy (lockout) for other than
this intended
purpose.
b. On or about June 8, 2011, at the electrical platform, employees used a
lock for the
control of electrical energy (lockout) which was not unique to the
employee performing
lockout.  Five different employees had possession of a key which opened
the lock.
Among other methods, one feasible and acceptable means of abatement would
be to
implement a lockout/tagout program which complies with article 120,
Establishing an
Electrically Safe Work Condition, of the National Fire Protection
Association (NFPA)
70E-2012, Standard for Electrical Safety in the Workplace.
