exposed to Falls and Struck-by hazards:
Located at 20 W. 60th Street, New York, NY
On or about July 29, 2011
a)An employee was exposed to falling approximately 30 feet while working
from
a Genie model AWP 40-S aerial work platform unit that was moved with an
employee on the raised platform and the outriggers removed.  Two employees
were injured when the aerial platform tipped over while it was being moved.
Abatement Note
Among other, a feasible method to eliminate or to materially reduce this
hazard
would be to:
1) Ensure the platform unit is not moved with the platform raised, and
2) Ensure outriggers are not removed or adjusted while the platform is
occupied
or raised, as detailed in the Genie Operators Manual, Part No. 82910, page
2.
NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER
IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS
ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY
OF $1,000.00 AS PER 29 CFR 1903.19.0
