above the adjacent
floor or ground level were not guarded by standard railings (or the
equivalent as specified in 29
CFR 1910.23(e)(3)(i) through (v)), on all open sides:
(a)Knouse Foods, Orrtanna, PA, Plant - Employees were exposed to falls of
approximately 25" to 30' from an open sided upper roof level on the
production
facility without any form of fall protection.  The area is routinely
accessed by
fixed industrial stairs to inspect equipment located on relief risers.
Condition
observed on or about March 2, 2012.

not obtain and
evaluate information regarding the contract employer's safety performance
and programs:
(a)Knouse Foods, Orrtanna, PA, Plant - The employer has used a primary
contractor
to perform repairs on the Process Safety Management covered anhydrous
ammonia refrigeration system. There is no record for an initial evaluation
of the
contractor's safety record and programs.
Abatement certification required within 10 days after abatement date. The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.

performance of
contract employers in fulfilling their obligations as specified in 29 CFR
191.119(h)(3):
(a)Knouse Foods, Orrtanna, PA Plant  - The employer did not perform
periodic
evaluations of the contractor they have had service their Process Safety
Management (PSM) covered anhydrous ammonia refrigeration system for 38
years.  There are no provisions to perform periodic audits in the
"Contractors"
section of the employer's PSM program.
Abatement certification required within 10 days after abatement date. The

shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.

response
to each of the findings of the compliance audit required by 29 CFR
1910.119(o)(1), and
document that the deficiencies had been corrected:
(a)Knouse Foods, Orrtanna, PA, Plant - The employer conducted two
compliance
audits one in 2008 and one in 2011.  Both audits identified a lack of
emergency
lighting.  The employer had not provided emergency lighting at the time of
the
inspection in 2012.
