Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized
hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall and struck-by hazards.
On or about December 13, 2011, and at times prior thereto, employees
assigned to work on the
monkey board were not provided an adequate means of safe egress from the
work platform.
Among other methods, one feasible and acceptable method to correct this
hazard is to install the
auxiliary escape in accordance with the American Petroleum Institute,
Recommended Practice
54, Section 6.10.1a.
