40.1-51.1(a) Code of Virginia:  The employer, Paul Davis Restoration, did
not furnish employment
and a place of employment which was free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to hazards
associated with the operational use of the Salem electric freight
elevator. The employer failed to
conduct routine inspections at intervals not longer than six (6) months,
to identify deficiencies of the
elevator.
One feasible and acceptable method, among others, to correct this hazard
would be for the employer
to conduct routine inspections such as that called for by ASME A17.1-1990,
Safety Code fo Elevators
and Escalators: Section 1001 Routine inspections and test of Freight
Electric Elevators.
Rule 1001.1  Inspection and Test Periods:
The routine inspection and test of passenger and electric freight
elevators shall be made at
intervals not longer than six (6) months.
The elevator was last inspected approximately four (4) years ago.s
