Labor and Employment Article, Section 5-104(a): The employer did not
furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:
(a)Date of Violation:02/11/14

Equipment:Sky Climber Alpha 1000 Hoist, Serial Number Illegible
Condition:The electric hoist motor malfunctioned while the suspension
scaffold was elevated
approximately 80 feet above the ground, leaving employees hanging from
lifelines and
requiring them to be rescued.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is for the
employer to service the suspension scaffold hoist motors at least annually
per manufacturer's
recommendations. Also, the employer should follow Service Bulletin #127,
issued in January 2002, pertaining
to the particular make and model of the hoist, which recommended the
addition of an emergency stop switch
element and wire to provide positive brake actuation, to be serviced by a
Factory Authorized Service Center.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
