29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Northwest corner of structure at roof level; On or about 3/23/11 and at times prior thereto, two employees without a means of fall protection were walking/working at distances of 8 or less feet from the roof edge while assisting a crane in hoisting/setting a large metal frame upon the roof deck.    Note: Abatement certification is NOT required for this item.

29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualification Requirements (paragraph (c) of this section) prior to giving any signals. This requirement must be met by using either Option (1) or Option (2) of this section.    a) Northwest corner of structure at roof level; On or about 3/23/11 and at times prior thereto, an employee not meeting the qualification requirements was signaling a crane that was hoisting/setting a large metal frame onto the roof deck.
