29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) JDCC Home Maintenance at 1458 Gaylord St., Denver, Colorado 80206: On or before September 22, 2009 the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on a roof with a fall hazard of approximately 9.5 feet without the use of a fall protection system.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a) JDCC Home Maintenance at 1458 Gaylord St., Denver, Colorado 80206: On or before 4/5/2011, the employer did not ensure that a portable extension ladder, used to access the roof of a garage, had side rails extending 3 feet above the upper landing, or was secured. Condition exposed 2 employees to a fall hazard of approximately 9.5 feet.
