29 CFR 1904.4(a):  The employer required by this part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that was work-related, a new case, and met one or more of the general recording criteria on the OSHA 300 Log or equivalent:  (a)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the employer did not record the following workplace injury on the OSHA 300 log for calendar year 2009:  (1)  On July 24, 2009, Vacation Relief  An employee received prescription medication for a contusion to the right foot while operating a pallet jack.  Abatement Note:  Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1904.29(b)(1):  The log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed accurately as required by the standard:  (a)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not completed accurately in that the date of injury recorded for case number two was incorrectly recorded as January 28, 2008.  The correct injury date for this recordable injury is January 26, 2008.    (b)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not completed accurately in that the date of injury recorded for case number four was incorrectly recorded as February 26, 2008.  The correct injury date for this recordable injury is February 22, 2008.    (c)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not completed accurately in that the date of injury recorded for case number five was incorrectly recorded as March 12, 2008.  The correct injury date for this recordable injury is March 11, 2008. 29 CFR 1904.29(b)(3):  The recordable injury or illness on the OSHA 300 Log was not recorded within seven calendar days of receiving information that a recordable injury or illness occurred:  (a)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not accurately updated to reflect the correct number of restricted work days based on receipt of information from the physician.  The number of restricted work days recorded for case number four was incorrectly recorded as 6.  The correct number of restricted work days for this recordable injury is at least 7.    (b)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not accurately updated to reflect the correct number of restricted work days based on receipt of information from the physician.  The number of restricted work days recorded for case number five was incorrectly recorded as 20.  The correct number of restricted work days for this recordable injury is 21.    (c)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the 2008 OSHA 300 Log was not accurately updated to reflect the correct number of restricted work days based on receipt of information from the physician.  The number of restricted work days recorded for case number eleven was incorrectly recorded as 64.  The correct number of restricted work days for this recordable injury is at least 159. 29 CFR 1904.30(a):  A separate OSHA 300 Log for each establishment that is expected to be in operation for one year or longer was not established:  (a)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 21, 2011, the employer did not maintain separate OSHA 300 Logs for each establishment.  Robinson Dairy has 3 separate establishments in Denver, CO:  manufacturing plant, transportation/warehouse, and administrative offices.  The OSHA 300 Logs for calendar years 2007 through 2009 had entries from multiple establishments. 1910.1200(f)(5):  The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  (a)  Robinson Dairy LLC at 2401 W 6th Avenue, Denver, CO 80204:  On and before June 30, 2011, the employer did not ensure that a container with HC-10 Chlorinated Kleer-Mor, located in the milk receiving bay, was labeled with the chemicals identity and hazards.  Employees use this chemical to hand clean stainless production equipment.  This condition potentially exposes employees to a chemical hazard.
