20 CFR 1910.22(a)(1): The employer failed to keep the floor of the workroom clean, and so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable:      a)  Robinson Dairy, LLC, 2401 W 6th Ave., Denver, CO 80204: On and before June 20, 2011 the employer did not maintain the floor of the plant workroom in a dry condition. A large volume of hot, processed milk was drained onto the floor near the access steps to the heating manifolds. This condition exposed employees to slips, trips, and falls hazards. The test record on the fire extingusiher in the freezer load-out and near the flammable cabinet and eyewash was not ispected each month. This condition exposed employees to fire hazard. This condition exposed employees to suffocation and entrapment hazards. 20 CFR 1910.212(a)(2): The employer did not affix guards on all machines where possible and secure elsewhere if for any reason attachment to the machine is not possible:     a)  Robinson Dairy, LLC, 2401 W 6th Ave., Denver, CO 80204: On and before June 20, 2011 the employer did not affix machine guards to the TGA 200 I durable box loaders (4X) with model no. RM-3FC-S and serial no. 152390907. The moving belts on the box loaders create a moving part hazard.     b)  Robinson Dairy, LLC, 2401 W 6th Ave., Denver, CO 80204: On and before June 20, 2011 the employer did not affix a machine guard to the box sorter in the cooler room. The sorter was operating with the guard off.The machine guard was on the ground, stating clearly " DANGER, this machine must not be operated without safety guard(s) in position. This condition exposed employees to cut/laceration hazards from moving belt part. 29 CFR 1910.176(b): The employer did not ensure that storage of material shall not create a hazard. bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:     a)  Robinson Dairy, LLC, 2401 W 6th Ave., Denver, CO 80204: On and before June 20, 2011 the employer did not ensure that the Mecalux and Unarco storage racks in the cooler, holding 2000 lbs per pallet, were not interlocked properly to prevent collapse. One rack resting on the third level was missing bolts on the left facing side. This condition exposed employees to crush-by hazards.
