29 CFR 1910.213(p)(4): Belt sanding machinery was not provided with a guard at each nip point where the sanding belt ran onto a pulley, to prevent the operator's hands or fingers from coming into contact with nip points:    (a)  On or about June 22, 2011, and at times prior, employees were exposed to serious injury while operating the Baldor belt sander without providing a guard at each nip point where the belt ran into the pulley to prevent the operator's hands from coming into contact with nip points.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed:    (a)  On or about June 22, 2011, and at times prior, employees were exposed to serious electrical injury from an electrical outlet located in the Pic Pac department of building 1, which had a knockout removed, exposing live electrical wires.
