29 CFR 1910.106(b)(6):  In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition:    (a)Knowles Enterprises Land Company, LLC, East Plateau Site @ My Way Ranch 17-2, Collbran, CO  81624: On or before June 29, 2011, the employer did not ensure the control of ignition sources while working in conjunction with other employers to empty and power wash the interior of storage/production tanks.  On June 29th, 2011, an employee was exposed to a struck-by hazard associated with flying projectiles after the flammable atmosphere inside the 300 barrel condensate tank ignited.

29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)Knowles Enterprises Land Company, LLC, East Plateau Site @ My Way Ranch 17-2, Collbran, CO  81624:   On or before June 29, 2011, the employer did not ensure procedures were developed, documented, and utilized to control potentially hazardous energy including but not limited to internal tank heaters and 4 inch ventilation lines interconnecting the 6 production/storage tanks prior to emptying and cleaning.  On June 29th, 2011, an employee was exposed to a struck-by hazard associated with flying projectiles after the flammable atmosphere inside the 300 barrel condensate tank ignited.
