29 CFR 1910.134(c): Respiratory protection program. This paragraph requires the employer to develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use. The program must be administered by a suitably trained program administrator. In addition, certain program elements may be required for voluntary use to prevent potential hazards associated with the use of the respirator: On or about 07/06/2011, the employer had not developed a respiratory protection program for employees required to use full facepiece respirators and employees using dust masks optionally.

29 CFR 1910.157(g)(4): The employer shall provide the training required in paragraph (g)(3) of this section upon initial assignment to the designated group of employees and at least annually thereafter.[45 FR 60708, Sept. 12, 1980; 46 FR 24557, May 1, 1981, as amended at 51 FR 34560, Sept. 29, 1986; 61 FR 9227, March 7, 1996; 67 FR 67964, Nov. 7, 2002]: On or about 07/06/2011, no fire protection program had been developed and written that would instruct employees on what actions to take in the event of an incipient-stage fire. Options include the following three: 1. All employees are trained on how to use a fire extinguisher upon initial employment and annually thereafter ; 2. Designated employees will be trained while those not trained will evacuate; or, 3. No employee will use an extinguisher; instead all employees will evacuate.
