29 CFR 1910.219(e)(1)(i):     Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    (a)  In the sawmill, the v-belt and pulley drive under the off-bearer conveyor by the head saw was not guarded.

29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a)  On or about July 3, 2011, the sawyer and his helper repaired and replaced the blades on the chipper and did not lock the main power disconnect switch off.  (b)  On or about June 12, 2011, the sawyer and his helper changed the saw blade on the big head saw and did not lock the main power disconnect switch off.  (c)  On or about July 3, 2011, the sawyer and his helper changed and worked on the debarker head and did not lock the main power switch off.
