29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) Ernest Rollins on or about June 29, 2011 at the address of 3555 Hanley Rd. Perrysburg, OH:  The employer failed to ensure an employee walking/working inside a hole cut into the structures roof deck was protected from a fall.  The exposed employee slipped from the structural member in which he was standing and fell approximately 20ft into a nearly empty pool inside the structure.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. \r\n\r\na)  Ernest Rollins on or about June 29, 2011 at the address of 3555 Hanley Rd. Perrysburg, OH: The employer did not ensure employees were protected from falls while performing roofing work on a low sloped commercial structure. Employees removing old roofing material from a 4/12 pitch roof were exposed to a 14\u0019 fall to the pavement below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  Ernest Rollins on or about June 29, 2011 at the address of 3555 Hanley Rd.
