29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:\r\n\r\n(a) Excavation located at UW River Falls- Johnson and Crabtree Resident hall project; Daily inspections of each excavation were not conducted by a competent person. The foreman only performed visual inspections of the excavation.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):\r\n(a) Excavation site located at UW River Falls- Johnson Hall; The slope angles of the excavations West and South sidewalls were greater than 1-1/2 to 1 (34 degrees). The West slope was approximately 41 degrees and the South slope was approximately 36 degrees.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): \\\\r\\\\n\\\\r\\\\n(a) Construction site located at UW River Falls, Johnson and Crabtree Residence halls; The employer did not develop a written hazard communication program or provide effective employee training about the hazards related to chemicals, such as, portland cement.
