29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a.  On or about July 21, 2011, at the above addressed jobsite, employees were performing tuckpointing operations from a tubular welded frame scaffold, approximately thirty (30) feet above the ground, that did not have the working level fully planked, thereby exposing employees to falls.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means:    a. On or about July 21, 2011, at the above addressed jobsite, employee were performing tuckpointing operations from a tubular welded frame scaffold with a height to base ratio greater than four to one (>4:1) that was not restrained from tipping, thereby exposing employees to falls.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platformand walkway shall be at least 18 inches (46 cm) wide:    a. On or about July 21, 2011, at the above addressed jobsite, employees performing tuckpointing operations from a tubular welded frame scaffold, approximately thirty (30) feet above the ground, were exposed to falls when the scaffold platform was not at least eighteen (18) inches in width.    The Erin Tuckpointing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(2), which was contained in OSHA inspection number 313934747, citation number 1, item number 1a and was affirmed as a final order on July 29, 2010, with respect to a workplace located at 4200 S. Indiana Avenue, Chicago, Illinois.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level:    a. On or about July 21, 2011, at the above addressed jobsite, employees performing tuckpointing operations from a tubular welded frame scaffold, approximately thirty (30) feet above the ground, were exposed to falls due to the lack of guardrails.    The Erin Tuckpointing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 313934747, citation number 1, item number 1b and was affirmed as a final order on July 29, 2010, with respect to a workplace located at 4200 S. Indiana Avenue, Chicago, Illinois.
