29 CFR 1926.250(b)(5): Materials were stored on scaffolds or runways in excess of supplies needed for immediate operations:    a.  Edgewood Estates, 58 Branch Tpk. #42, Concord, NH - On 07/25/11 a stack of shingles not needed for immediate operations was stored on the platform of a ladder jack scaffold. It was placed so that anyone stepping onto or off of the ladder accessing the scaffold would have to step on it.

29 CFR 1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a.  Edgewood Estates, 58 Branch Tpk. #42, Concord, NH - On 07/25/11 an employee who used the ladder jack scaffold to access the roof was not protected from falling 15 ft by a personal fall arrest system.

Edgewood Estates, 58 Branch Tpk. #42, Concord, NH - On 07/25/11 an employee performing roofing work was not provided protection from a fall of 18.8 ft.
