On July 21, 2011, during roofing activities, an effective and site specific safety and health program was not implemented on the worksite. Hazards observed, to which employees were exposed, reflect deficiencies in site inspections, training, and enforcement of federal safety and health regulations.

29 CFR 1926.501(b)(3):     Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:    On July 21, 2011, during roofing activities, the employer allowed employees to work at a hoisting area on a roof that was approximately 30 feet high and did not ensure that the employees were protected from falls.
