29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level: a) On or about October 6, 2010 through May 19, 2011, the employer did not ensure that it was determined if any employee potentially exposed to lead during activities related to cartridge case sorting in the basement of 429 Forest Avenue in Oak Park, Illinois, were exposed at or above the action level. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1025(l)(1)(i):\r\n\r\nEmployee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025:\r\n\r\na) On or about October 6, 2010 through May 19, 2011, the employer did not ensure that all employees with a potential airborne exposure to lead during activities related to cartridge case sorting in the basement of 429 Forest Avenue in Oak Park, Illinois, were informed of Appendices A and B of the Lead standard, 29 CFR 1025.\r\n\r\nIn accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
