Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical hazards.    a) On or about August 9, 2011, the employer did not ensure that employees were protected from electrical hazards in the following locations:    1) Roller Room Area, electric motor (440 volts) did not have strain relief.  2) Maintenance/garage, power washer did not have strain relief.  3) Maintenance/garage, metal receptacle had a missing knockout.  4) Maintenance/garage, electrical switch to the 300 lbs. lift, not the proper switch.  5) Maintenance/garage, east wall, breaker box was not labeled.  6) Maintenance/garage, emergency safety switch did not have sufficient access was blocked by metal cabinet.  7) Pump Room Area, missing cover on the electric motor to pump (copper wires exposed).

29 CFR 1928.57(c)(2)(i): The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets, and idlers were not guarded.    a)  On or about August 9, 2011, the employer did not ensure that employees were protected from amputation injuries while working with and near the following machines and areas:    1) Roller Room, roller machine (flake maker) did not have an adequate guard on the  belts and pullies (main drive).    2) Maintenance/Garage Area, air compressor (Speedline) did not have a guard on the belt and pullies.    3) Roller Room, two roller machines (flake makers) did not have guards on the chains and sprokets.    4) Maintenance/Garage Area, the drill press did not have a guard on the drill bit.    5) Maintenance/Garage Area, the hydraulic press did not have a guard on the point of operation.
