29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:\\r\\n\\r\\na)  On or about August 11, 2011, employees were exposed to injury while using a portable ladder to access a roof where the ladder did not extend at least 3 feet above the roof surface.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)\\r\\n\\r\\na)  On or about August 11, 2011, employees were exposed to falls of up to 21 feet while performing roofing work without fall protection.  Employees were also exposed to falls in excess of 6 feet through open skylights on the roof.\\r\\n\\r\\nWilcox Roofing, Inc. was previously cited for a violations of this Occupational Safety and Health Standard, or its equivalent, 29 CFR 1926.501(b)(13) contained in OSHA inspection #315016733, Citation 1, Item 1 issued on November 9, 2010 and became a final order on December 2, 2010 with regard to a worksite located at 4830 Sand Hill Dr., Colorado Springs, CO 80923.
