29 CFR 1910.23(a)(8)(ii):     Every floor hole into which persons could accidentally walk were not guarded by a floor hole cover of standard strength and construction:    Employees cleaned the top of coal bunker 4 , E2 & E3, and were exposed to 2 floor holes, approximatley 4 inches in diameter and a square hole approximatley 12 inches in length and 5 inches in width, without guarding or covers.

29 CFR 1910.1200(e)(1):     Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):     A written hazard communication program wasn't developed for employees clean coal plants with vacuum truck and pressure wash truck and other chemicals employees work with at the shop.
