29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    (a)  In Building 122, the A Frame Crane Over the Shears - The employer had not guarded the chain and sprocket on the moving leg of the A Frame Crane.  This deficiency was noted on or about August 30, 2011.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a)  In Warehouse #316, At the Plug Connection for the Tempe Fan - The plug had pulled away from the cable attached to the Tempe Fan due to strain relief not being adequate.    (b)  In Warehouse #316, On the Power Cable running into the Knife Switch Box - The cable had pulled away from the strain relief on the box.     Both deficiencies were noted on or about August 30, 2011.
