This deficiency was noted on or about August 30, 2011.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a)  In Warehouse #316, At the Plug Connection for the Tempe Fan - The plug had pulled away from the cable attached to the Tempe Fan due to strain relief not being adequate.    (b)  In Warehouse #316, On the Power Cable running into the Knife Switch Box - The cable had pulled away from the strain relief on the box.     Both deficiencies were noted on or about August 30, 2011.
