Each employee on ramps, runways, and other walkways shall beprotected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems.      a.    On the roof where employees were walking out to the roof to throw materials from the roof into the dumpster, the employer did not provide a fall protection system such as, but not limited to, guardrails thereby exposing employees to a 13'5" fall hazard.

Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safetymonitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted.    a.   On the roof of the mini mall where employees were tearing off a commercial roof and then relaying another roff, the employer did not install a fall protection system around the entire work area.

29 CFR 1926.502(f)(1)(i):     When mechanical equipment is not being used, the warning line shall be erected not less than 6feet (1.8 m) from the roof edge.      a.   On the roof where tear off of old roofing was completed and new roofing was being installed, the warning line on the left side of the back of the building was erected three feet from the edge thereby exposing employees to a 13'5" fall hazard.

29 CFR 1926.503(a)(1):     The employer shall provide a training program for each employee who might be exposed to fallhazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards.    a.    On the roof of the building where employees worked at heights which exceeded six feet, the employer did not provide fall protection training thereby exposing employees to a fall hazard.
