29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: (a) Wojcik Builders, Inc. on or about September 01, 2011, at the address of 7000 Kalahari Dr. Sandusky, OH: The employer failed to ensure employees working from the Genie model # S-40 aerial lift were adequately protected from falls. Employees did not have their lanyards attached to the basket of the stated aerial lift while the basket was elevated. The employees were exposed to an approximately 10 ft fall to the ground below.
