29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about September 9, 2011, employees at Premier Tool Works, Inc., were required to wear a half-mask, negative pressure, respirator while spray painting metal parts. The employer did not establish and implement a written respiratory protection program for these employees. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about September 9, 2011, employees at Premier Tool Works, Inc., were required to operate a forklift truck in order to load/unload trucks. The employer did not ensure that each operator was competent to operate a powered industrial truck safely by training and evaluating the employees in accordance with paragraph (l). Abatement documentation is required for this item in accordance with the requirements of 29 CFR 19103.19(d).
