29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:           a)  Guadalupe Moreno Construction at 2474 Wolf Creek Dr., Brighton, Colorado 80601:  On September 14, 2011 the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on a roof with a fall hazard of approximately 14.8 feet without the use of a fall protection system.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").    Guadalupe Moreno Construction was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection #312142011, Citation 2, Item 2, issued on July 14, 2009, and became final order on August 6, 2009 with respect to a workplace located at 4602 & 4600 Plettner Ln., Evergreen, CO 80603.
