29 CFR 1910.272(g)(1)(i): The employer did not issue a permit for entering bins, silos or tanks in the grain handling facility before the employees entered the bins, silos, or tanks: An employee was exposed to cave-in hazards from falling grain and struck by hazards from a Brock 10 inch ABC Model Commercial Bin Sweep while working in a 108 foot 5 finch diameter GSI bin. The employee was also exposed to possible explosive atmospheres and/or lack of oxygen while inside the bin. The employer did not issue a permit for the employee entering the bin.

An employee was exposed to being struck by a Brock 10 Inch ABC Model Commercial Bin Sweep while conducted services operations in a 108 foot 5 inch diameter GSI bin. The Brock Bin Sweep was not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.
