29 CFR 1910.272(g)(1)(i):     The employer did not issue a permit for entering bins, silos or tanks in the grain handling facility before the employees entered the bins, silos, or tanks:    An employee was exposed to cave-in hazards from falling grain and struck by hazards from a Brock 10 inch ABC Model Commercial Bin Sweep while working in a 108 foot 5 finch diameter GSI bin.  The employee was also exposed to possible explosive atmospheres and/or lack of oxygen while inside the bin.   The employer did not issue a permit for the employee entering the bin.

29 CFR 1910.272(g)(1)(ii):     Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.     An employee was exposed to being struck by a Brock 10 Inch ABC Model Commercial Bin Sweep while conducted services operations in a 108 foot 5 inch diameter GSI bin.  The Brock Bin Sweep was not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.

29 CFR 1910.272(g)(6):     Employees at the grain handling facility entered bins, silos, or tanks underneath a bridging condition, or where a buildup of grain products on the side could fall and bury them:    While conducting maintenance operations in a pit a Brock 10" ABC Model Commercial Bin Sweep, an employee was exposed to build up of corn which fell on the employee on September 28, 2011.    This was located inside a 108 foot 5 inch (in diameter) GSI bin.

29 CFR 1910.272(g)(1)(iii):     The atmosphere within a bin, silo, or tank in the grain handling facility was not tested for the presence of combustible gases, vapors, and toxic agents prior to employee entry:    An employee was exposed to possible combustible gases, vapors, and toxic agents prior the employee entering a 108 foot 5 inch diameter grain bin.   The bin was not tested for the presence of these hazards.
