The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a. On or about September 29, 2011, employees performing roofing operations were not trained to recognize the hazards of falls and the procedures to be followed in order to minimize the hazards, thereby exposing employees to falls.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about September 29, 2011, at the above addressed jobsite, on the north section of the 6:12 pitched roof, employees receiving roofing materials, were not protected from falling, thereby exposing employees to fall hazards of twenty-seven (27) feet. b. On or about September 29, 2011, at the above addressed jobsite, on the north section of the 8:12 pitched roof of the entrance, an employee receiving roofing materials, was not protected from falling, thereby exposing the employee to fall hazards of twenty-three (23) feet.
