29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    The employer did not provide abatement documentation for the following cited items:    1.  Employees did not wear fall protection while working on a commercial building at a fall height greater than 25 feet.    2.  Employees were not provided training on fall protection procedures, how to recognize fall hazards, or how to mitigate those hazards.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrestsystems.    Employees were working from a steep sloped roof at height greater than 6 feet with no fall protection.     The employer does not require the use of any form of fall protection during roofing work. This violation was last observed at commercial building at the address of 2727 Park Place Lane WI 53547, where thirty-five employees were not protected from fall hazards of greater than 25 feet to the ground.  To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than 6 feet high without adequate fall protection.     Pursuant to 29 C.F.R. 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from fall hazards.      This employer has been previously cited for this practice that includes:    GTO Contractors LLC was previously cited for violation of this Occupational Safety and Health standard 29CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection #312559156, Citation 1, Item 1 issued on 1/4/11 with regards to a workplace located at 1225 Oakcrest, Appleton, WI 54911.    GTO Contractors LLC was previously cited for violation of this Occupational Safety and Health standard 29CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection #312558240, Citation 1, Item 1 issued on 9/13/10 with regards to a workplace located at 1223 Stead Drive, Menasha, WI 54952.    GTO Contractors LLC was previously cited for violation of this Occupational Safety and Health standard 29CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection #310763511, Citation 1, Item 2 issued on 10/9/07 with regards to a workplace located at 4810 Dream Lane, McFarland, WI 53558.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fallhazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards.    Employees who were exposed to fall hazards were not trained in fall protection procedures to recognize and minimize those hazards.    GTO Contractors LLC was previously cited for a violation of this occupational safety and health standard or it equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection 310763511, citation number 1, item number 3 and was affirmed as a final order on October 23, 2007, with respect to workplace located at 4810 Dream Lane, McFarland, WI 53558.
