29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    Employees working on a steep-sloped roof at a height greater than six feet were doing so without fall protection.    The employer does not require the use of any form of fall protection during roofing work. This violation was last observed at a commercial building at the address of 8628 Greenway Boulevard, Middleton, WI 53562, where five employees of GTO Contractors LLC were not protected from fall hazards of greater than 25 feet to the ground. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than 6 feet in elevation without adequate fall protection.     Pursuant to 29 C.F.R. 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from fall hazards.      This employer has been previously cited for this practice:    GTO Contractors LLC was previously cited for violation of this Occupational Safety and Health standards 29CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection #312559156, Citation 1, Item 1 issued on 1/4/11 with regards to a workplace located at 1225 Oakcrest, Appleton, WI 54911.    GTO Contractors LLC was previously cited for violation of this Occupational Safety and Health standards 29CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection #312558240, Citation 1, Item 1 issued on 9/13/10 with regards to a workplace located at 1223 Stead Drive, Menasha, WI 54952.
